Hospice Registered Nurse Case ManagerJob Type: Full-TimeSetting: Home Health / HospiceWork Location: Kansas City, MO (In person/Field Base)Pay Range: $85,000 - $90,000 per yearJob OverviewWe are seeking a compassionate and experienced Hospice Registered Nurse Case Manager to provide high-quality hospice care to patients in their homes and senior living settings.This is a full-time, field-based nursing role where the RN will manage an assigned hospice patient caseload, coordinate care plans, support patients and families, and work closely with the interdisciplinary care team. The ideal candidate has prior hospice experience, strong clinical judgment, and is comfortable traveling throughout the day to visit patients in their homes.This position is with a well-established home health and hospice organization that has served patients for many years and offers long-term stability, strong benefits, and a supportive care environment.Key ResponsibilitiesProvide skilled nursing care to hospice patients in their homes or senior living facilitiesManage an assigned hospice patient caseload and coordinate individualized plans of carePerform patient assessments, monitor changes in condition, and provide appropriate interventionsEducate patients and families on hospice care, symptom management, medications, and end-of-life supportCollaborate with physicians, aides, social workers, chaplains, and other members of the interdisciplinary teamComplete timely and accurate documentation using electronic chartingCommunicate regularly with patients, families, caregivers, and the care team regarding patient needs and changes in statusTravel throughout the service area to complete daily patient visitsRequirementsActive Registered Nurse license in the statePrior hospice experience requiredComfortable providing care in home-based and senior living settingsAbility to manage a hospice caseload independentlyStrong communication, documentation, and patient education skillsMust be willing and able to travel throughout the day to complete patient visitsMust be a U.S. citizen or green card holderBenefitsCompetitive annual salaryHealth insuranceDental insuranceVision insuranceHealth Savings AccountFlexible Spending Account401(k) retirement planPaid time off, starting at 3 weeks per year and increasing with tenureShort-term disabilityLife insurancePaid mileageElectronic chartingReferral programSchedule and Travel ExpectationsThis is a full-time position. This is not a traditional shift-based role. The RN will visit multiple patients per day in their homes or senior living facilities. Most patients will be located near each other, but some visits may require a 20- to 40-minute drive.Travel is a regular part of the role and is included in the salary, with additional mileage reimbursement provided.
