Description
HACM is soliciting Proposals from qualified firms and organizations to provide Supportive Services for the residents at the Broadway Project and/or the Fairview Project, as set forth below:
- Case management
- Behavioral health services
- Physical health services
- Assistance obtaining benefits and essential documentation
- Life skills for daily living, rehabilitation, and independence
- Total Full-Time Equivalent ("FTE") staff positions (half-time is 0.5) – Case manager ratio is 15:1 for residents experiencing Chronic Homelessness and 25:1 for residents at-risk of homelessness.
