Schedule: Monday- Friday 10am-7pm
Location: 120 CYPRESS EDGE DRIVE, Palm Coast, 32164

May travel to the other Palm Coast location at 60 Memorial Medical Parkway

Job Description:

• Practices principles of employee and patient safety during daily job activities. Supports consumer marketing and public awareness through participation in community events, educational lectures, and other extensions of clinical expertise to the community.
• Implements individualized treatment plans based on physical therapy evaluations and through consideration of practice standards, recommended techniques, insurance guidelines, and patient needs.
• Modifies treatment plans as necessary based on ongoing assessments in consultation with the physical therapist.
• Collects patient data and communicates with the physical therapists regarding discharge recommendations when maximum benefit has been achieved.
• Documents patient care accurately, completely, and timely using measurable and objective terminology, following standards of practice, insurance guidelines, and department/hospital policies.

QUALIFICATION REQUIREMENTS:

• Associate (Required)
• Basic Life Support – CPR Cert (BLS) – RQI Resuscitation
• Quality Improvement, Physical Therapy Assistant (PTA) – EV Accredited Issuing Body, Physical Therapy Assistant – TEMP (PTA-TEMP) – EV Accredited Issuing Body

Pay Range: $21.56 – $40.09
