Position Title: Licensed Practical/Vocational Nurse Department: Health and Wellness
Reports to: Director of Health & Wellness FLSA Designation: Non-Exempt
Locations:
- Grand Living at Tamaya – Jacksonville, FL
- Grand Living at Lake Mary – Lake Mary, FL
POSITION PURPOSE:
Responsible for overseeing and enhancing the overall Resident and family experience through the provision of comprehensive nursing leadership to the staff working within our Independent and Assisted Living care environment. Assists with assuring the general health and well-being of residents through the delivery of comprehensive and high-quality services. This is to be handled in The Sterling Touch® friendly, courteous, helpful, timely and professional manner, resulting in a very high level of Resident satisfaction. This position is under the general guidance of the Director of Health and Wellness in accordance with Community standards, processes, procedures, practices, and philosophy.
Essential Functions:
- Participate in Start Sterling! line up each day.
- Greet the Residents, guests, fellow employees, and vendors in a friendly and courteous manner.
- Providing nursing services including medication administration and delegation, as well as provision of care and services to residents who have orders by their attending physician to be admitted to the community for Assisted Living care per State regulations.
- Ensure the evaluation and clinical competence of assigned nursing personnel, assisted living aides, and medication aides by participation in orientation, education, teaching and supervision of staff.
- Conducts frequent rounds to evaluate effectiveness of care delivery.
- Provide direct nursing services to residents in accordance with State regulations within Assisted Living community, including observing, assessing, and reporting resident condition changes.
- Receive and transcribe physician orders, assuring the implementation of care plans, investigating, and documenting all accidents and incidents, medication administration, treatment orders, electronic chart notes, admission/discharge/transfer paperwork, working with other health care professional and community staff to provide timely and responsive care services to our residents
- Use best-practice nursing judgment in monitoring residents’ changes in condition and take appropriate steps to address changes, notify Medical Providers and resident representatives of change in a resident’s condition.
- Responsible for ensuring that staffing is adequate at all times to meet resident personalized service plans and needs.
- Assists Director of Health and Wellness to ensure medical supplies are ordered and available in adequate quantity for the health and wellness department.
- Supports, cooperates with, and implements specific procedures and programs for:
o Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security.
o Confidentiality of all data, including resident, employee, and operations data.
o Quality Assurance and compliance with all regulatory requirements.
Other:
Employees are required to work a flexible schedule which includes, rotating weekend and holiday shifts.
Upon employment, all employees are required to fully comply with Grand Living’s standards, procedures, and processes, including all government and industry regulations, for the safe and efficient operation of the Community. This includes regular attendance as well as wearing the prescribed uniform and maintaining a neat, clean, and well-groomed appearance. Employees who fail to meet the Grand Living expectations in all areas will be subject to disciplinary action, up to and including termination of employment.

SPECIFIC POSITION KNOWLEDGE, SKILL, AND PHYSICAL REQUIREMENTS
The individual must possess the following knowledge, skills, and abilities:
- Ability to read, write and understand the English language in order to educate, train and supervise staff; meet with residents and their families to facilitate appropriate assessments and development of personalized care plans; utilize and train others on operational software for delivery of care documentation and record keeping; and communicate with other employees effectively.
- Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all residents in a safe, secure environment.
- Ability to stand, walk, climb stairs, crouch, bend, stoop, reach, grasp, push, pull and work continuously without sitting for extended periods of time.
- Ability to lift or carry up to thirty (30) pounds.
QUALIFICATIONS
Education:
- Associates Degree in Nursing, required.
Experience:
- Three or more years’ nursing experience, preferred.
- Assisted living or home health care experience, preferred.
Licenses/Certificates:
- Must be a Licensed Practical/Vocational Nurse in the state of employment, required.
Lake Mary (Grand Living at Lake Mary)
Part-Time Positions Available
Shifts:
Morning Shift: 7:00 AM – 3:30 PM
Evening Shift: 3:00 PM – 11:30 PM
Rotating weekends required.
Jacksonville (Grand Living at Tamaya)
Full-Time and Part-Time Positions Available
Shifts:
Part-Time: 3:00 PM – 11:00 PM
Part-Time: 11:00 PM – 7:00 AM
Full-Time: 3:00 PM – 11:00 PM
Rotating weekends required.
We welcome new graduate nurses and will also consider Registered Nurses (RNs) for these positions. Candidates with clinical rotations in skilled nursing or senior living are preferred, though not required. Experience in skilled nursing, assisted living, or long-term care is highly valued.
